Health Science Simulation Center Space Request Form

Please complete the following form to request space in the HSSC.  Please schedule enough time for set-up and cleanup of the HSSC.  If your request includes a manikin-based simulation and/or a standardized patient please complete the appropriate forms. 

You will receive a copy of your submission via email once the form has been completed as a receipt.  Once the HSSC has processed your request, we will contact you directly via email with a confirmation of your reservation.  Please review these emails carefully for accuracy.  If you must cancel your scheduled visit, please do so a minimum of 24 hours in advance.  If you have any questions, please contact:
